Allergy patients: Antihistamines as well as some other medications can interfere with skin testing results and need to be discontinued for a period of time prior to your visit with us. Please refer to the Skin Testing Medicines Not to Take Form for more specific information.

If you currently have an appointment scheduled with one of our physicians, you may download the appropriate history form from the list below. Please use the criteria listed to assist you in downloading the proper form. Forms should be completed prior to your appointment date and time.

	Allergy Survey Sheet	Dr Hartel - all ages Dr. Holzhauer - all ages Dr. Kubicka - 11 years and up
	Patient's Personal History Form	Dr. Quaidoo - 16 years and up Dr. Pulver - all ages Dr. Deane - 18 yrs and up
	Pediatric History Form	Dr. Quaidoo - up to 15 yrs., inclusive Dr. Kubicka- up to 10 yrs., inclusive Dr. Deane - up to 17 yrs., inclusive
	Rheumatology Form *	Drs. Condemi, Deane, Quaidoo 18 yrs. and up
	Hives Form	All physicians, in addition to a history form from above.
	Nasal Treatments Nasal Saline ENT Solution Upper Respiratory Infection	All physicians.

*Prior to making a rheumatology consult, your PCP must send a letter stating why you need to be seen along with office notes and lab work.
